Record in Q. 25 the total number of household members who have had any of the listed diseases. In Q. 26, ask the name (or confirm if you already have recorded the name in Q. 23) of each person who have had the disease; then locate in Col. (2) the name of the person and copy the person's line number in Col. (1). Enter the line number and name for Person 1, Person 2, etc.; make sure to record the line numbers in ascending order. If there are more than twelve persons, use another questionnaire. Check for the consistency of the total number in Q. 25 and the number of persons listed in Q. 26.
Read out each disease in the list for each household member recorded in Q. 26. Circle all appropriate codes in Q. 27. After asking Q. 27 for all persons listed in Q. 26, count the number of circled codes for each disease and compare this with the number in Q. 23(B). If the numbers are not consistent, verify from the respondent and make the necessary corrections.
